Book your first meeting appointment with an Accessibility Advisor by calling 604-599-2828.
Step 2
The Accessibility Advisor will email you a copy of our “first meeting” form along with a MS teams link (if the meeting is online). This form is designed to gather only the essential information required to start the academic accommodation planning process while you are at KPU.
The "first meeting" form will ask you to:
- Read and sign the “Confidentiality Commitment” (section 2) part of the form.
- For sections 3-5 you have a choice: you can fill in the form before your first meeting with an Accessibility Advisor, or you can complete it during your first meeting if you haven’t had the chance to complete it prior to the first meeting.
- It is preferred to have the form sent to the Accessibility Advisor via email but if that method doesn’t work, then you can either drop it off or bring it to the first meeting (if the meeting is in-person).
Step 3
Documentation.

Please bring any relevant medical and supplemental (such as an Individual Education Plan - IEP) documentation to your first meeting.
Documentation requirements:
- Documentation must be from a qualified medical assessor with training, and expertise in diagnosing a disability.
- Documentation should indicate the diagnosis/diagnoses and include an explanation of the functional impact in a post-secondary setting.
- Documentation must include the full name, signature, credential and registration/certification of the qualified assessor.
- Medical documentation should be no more than three (3) years old.
- For learning disabilities no more than five (5) years should have passed between the completion of a psycho-educational assessment and registration with Accessibility Services, or it should have been conducted at the age of 18 or later.
- If you don't have any existing medical documentation, please speak with an accessibility advisor during the first meeting to discuss what medical documentation is required.
Examples of medical documentation:
Hard of Hearing: Audiogram/Audiology report
Low Vision: Ophthalmological report
Deaf or Blind: Doctor's letter
Learning Disability: Psycho-educational assessment
Mental Health: Psychiatrist's report
Medical Conditions: Specialist's report
